Margaret

This review was commissioned following the tragic death of Margaret, who was killed by her adult son during a serious mental health crisis. The Community Safety Partnership extends its sincere condolences and sympathies to Margaret’s family, friends and all those impacted by these tragic events. We recognise the significant loss experienced by those who knew and cared for Margaret and acknowledge the continuing effect such circumstances can have on individuals, families and communities.

Domestic Homicide Reviews are undertaken to understand what can be learned from such events and to identify opportunities for agencies to work together more effectively in the future. The review considered the involvement of partner organisations and highlighted learning to strengthen safeguarding practice, risk assessment and multi-agency working. Partner agencies have worked collaboratively to implement the recommendations arising from the review, with actions now embedded or progressing to improve services, strengthen partnership working and reduce the risk of similar harm occurring in the future.
