This Domestic Homicide Review was commissioned following the death of Sarah.
The Community Safety Partnership extends its heartfelt condolences to Sarah's family, friends and all those affected by her death. We recognise the profound impact that such a loss can have on loved ones, communities and professionals involved.
Domestic Homicide Reviews are undertaken to help agencies understand the circumstances surrounding a death where domestic abuse may have been a factor, and to identify opportunities for learning and improvement. The purpose of these reviews is not to apportion blame, but to strengthen the way organisations work together to safeguard individuals and respond to risk.
This review considered the involvement of partner agencies and identified learning to enhance safeguarding practice, risk assessment and multi-agency collaboration. Organisations involved have worked together to implement the review's recommendations, with actions either completed or progressing, to improve services, strengthen partnership working and help reduce the risk of similar harm occurring in the future.
Following careful consideration, and with the agreement of the Home Office, it was decided not to publish the full review report in order to minimise any potential impact on Sarah's family. However, in the interests of openness, transparency and shared learning, a summary learning briefing is available below.
