This review was commissioned following the tragic death of Samantha, who died by suicide. The Community Safety Partnership extends its sincere condolences to Samantha’s family, friends, and all those affected by these circumstances. We recognise the profound loss experienced by those who knew and cared for Samantha and acknowledge the lasting impact that such events can have on individuals, families, and communities. The timing of publication has been carefully considered to avoid significant anniversaries and family occasions wherever possible, in order to minimise any additional distress.

This Domestic Homicide Review was undertaken to identify learning and understand whether there were opportunities for agencies to work more effectively together to support and safeguard Samantha. The review examined the involvement of partner organisations and identified areas for learning to strengthen safeguarding practice, risk assessment, information sharing, and multi-agency working. Partner agencies have worked collaboratively to address the findings and recommendations arising from the review, with agreed actions either completed or progressing through implementation plans designed to improve services, strengthen partnership working, and reduce the risk of similar harm occurring in the future.
